Low Estrogen And Low Progesterone: Why This Is A Different Problem

Most hormonal advice for women is written about a ratio problem: estrogen high relative to progesterone. Reduce the estrogen, support the progesterone, rebalance the two.

None of that applies if both are low. And when women in that situation follow ratio-based advice — cutting estrogen further with clearance supplements and cruciferous protocols — they get worse rather than better, then conclude that hormonal approaches do not work for them.

Low estrogen with low progesterone is a distinct picture with distinct causes, and telling it apart from the ratio problem is the single most useful thing you can do before acting.

Three patterns, not one

High estrogen, low progesterone. The classic “estrogen dominance” pattern. Anovulatory cycles with estrogen continuing more or less normally. Signature: heavy bleeding, breast tenderness, premenstrual anxiety, fluid retention. Covered in how to tell whether estrogen dominance applies to you.

Normal estrogen, low progesterone. Luteal phase insufficiency with an otherwise functioning cycle. Signature: premenstrual spotting, short luteal phase, sleep disruption without much else.

Low estrogen, low progesterone. The whole axis is down. Not a ratio problem — an output problem. Signature is completely different, and it is what this article is about.

The distinguishing question is simple: are you dry and thin, or heavy and swollen? Estrogen builds tissue and holds fluid. Where estrogen is low, tissues thin, mucous membranes dry, bleeding gets lighter. Where estrogen is high relative to progesterone, everything is heavier, fuller and more tender. Those are opposite pictures and they are hard to confuse once you know to look.

What low estrogen with low progesterone feels like

Vaginal dryness, and discomfort during sex. Often the earliest and most specific sign. Estrogen maintains the vaginal epithelium and its blood supply. This one rarely appears in the high-estrogen pattern at all.

Periods getting lighter and shorter, or stopping. Less estrogen means less lining is built. If your bleeding has become notably scanty, that points away from a ratio problem.

Hot flushes and night sweats. Driven by low estrogen narrowing the thermoneutral zone. Their presence is a reasonably strong indicator that estrogen is genuinely low rather than merely unopposed.

Joint aches without injury. Estrogen has anti-inflammatory action and receptors in joint tissue. Stiff hands and knees in the morning is a recognised feature.

Skin thinning and dryness, more fragile, slower to heal. Estrogen supports collagen; loss is measurable in the first years after menopause.

Recurrent urinary tract infections or new bladder urgency. The urethra and bladder trigone are estrogen-sensitive tissues, and thinning affects them as much as the vagina.

Low mood and flat affect rather than the anxious, wired quality of luteal progesterone deficiency. Different texture, and worth distinguishing.

Poor sleep, from both directions — no progesterone to promote sleep maintenance, and vasomotor symptoms causing waking.

Bone loss, which is silent, and the reason this pattern matters medically rather than only symptomatically.

Why both would be low

Late perimenopause and menopause. The commonest cause. Early perimenopause is characterised by erratic and sometimes high estrogen; late perimenopause is genuine decline in both. The transition from one to the other is why women describe their symptoms changing character entirely over a few years — heavy and tender at 43, dry and flushing at 49.

Functional hypothalamic amenorrhoea. The hypothalamus suppresses reproductive function under perceived energy scarcity: under-eating, over-training, very low body fat, severe stress, or a combination. The entire axis goes quiet, so both hormones fall together. Extremely common in athletic women and in women who have been dieting persistently, and frequently missed because the woman looks healthy and the symptom — periods stopping — gets attributed to stress in a vague way rather than diagnosed.

Primary ovarian insufficiency. Ovarian function ending before 40, affecting roughly 1 in 100 women. It needs diagnosing rather than assuming, because untreated it carries meaningful long-term bone and cardiovascular consequences and there is an established treatment.

Post-surgical. Removal of both ovaries produces an abrupt version rather than a gradual one, and the symptoms are correspondingly more severe. Hysterectomy with ovaries retained is a different situation, discussed in what happens to your hormones after hysterectomy.

Hyperprolactinaemia and thyroid disease. Both can suppress the axis. Both are simple tests.

Certain medications, including GnRH agonists and antagonists used for fibroids and endometriosis, which suppress estrogen deliberately.

Why the usual advice makes it worse

This is the practical point of the whole article.

Estrogen-clearance protocols — DIM, calcium D-glucarate, I3C, aggressive cruciferous intake — are designed to help the body process and eliminate estrogen faster. Applied to a woman whose estrogen is already low, they push in exactly the wrong direction and can worsen dryness, joint pain and mood.

Similarly, “estrogen detox” diets, aggressive fibre protocols aimed at gut clearance, and heavy restriction all reduce estrogen exposure or availability. If your problem is insufficient production, none of that helps.

And if the cause is hypothalamic — under-eating and over-training — then more restriction and more exercise is the precise opposite of the treatment, which is eating more and training less. Women in this situation frequently double down on discipline and get sicker.

Testing, and its limits

Estradiol and progesterone together, plus FSH. In menopause, FSH rises as the pituitary shouts at ovaries that are no longer responding, so a persistently high FSH with low estradiol is the characteristic pattern.

Two caveats. In perimenopause these values swing week to week, so a single draw is a snapshot rather than a diagnosis — guidance in several countries advises diagnosing perimenopause over 45 on symptoms rather than bloods. And in hypothalamic amenorrhoea the picture is different: FSH and LH are low rather than high, because the problem is upstream in the brain rather than in the ovary. That distinction is diagnostically important and often missed.

Add TSH, free T4 and prolactin to exclude the other suppressors.

What actually helps

If the cause is hypothalamic: eat more and train less. Unglamorous and it is the treatment. Restoring energy availability restores the axis, often within months. There is no supplement version of this.

If the cause is menopause or ovarian insufficiency: hormone therapy is the intervention that replaces what is missing. For primary ovarian insufficiency it is not optional in the way it is for typical menopause — it is generally recommended until the average age of natural menopause, for bone and cardiovascular protection. Worth knowing if you have been told you are “just early”.

Vaginal estrogen for local symptoms. Low systemic absorption, effective for dryness and recurrent urinary infections, and usable by many women who cannot or prefer not to take systemic hormone therapy.

Resistance training and adequate protein. Bone and muscle loss accelerate when estrogen falls, and this is the intervention with the most leverage over both, as set out in why muscle loss is the real metabolic problem.

Calcium, vitamin D, and knowing your bone density, since the bone consequence is silent until it is not.

A specific caution about supplements in this pattern, because it is the opposite of the usual advice. If both hormones are low, anything marketed for “estrogen balance” or “estrogen detox” is aimed at reducing an estrogen load you do not have, and it can make dryness, joint pain and mood worse. Check what a product is actually doing before taking it. The short version

Low estrogen with low progesterone is an output problem, not a ratio problem, and the two need opposite approaches. Ask whether you are dry and thin or heavy and swollen — dryness, lighter periods, hot flushes and joint aches point to both being low. The usual causes are late perimenopause, hypothalamic suppression from under-eating and over-training, and primary ovarian insufficiency. Estrogen-clearance protocols make this pattern worse, and if the cause is hypothalamic, more discipline is the opposite of the treatment. Test estradiol, progesterone and FSH together, and note that low FSH points upstream to the brain rather than to the ovary.
