Gut health rarely features in fibroid conversations — yet the gut plays a direct role in estrogen metabolism and excretion, making it more relevant to fibroid management than most people realise.

How the Gut Affects Estrogen Levels

Estrogen undergoes enterohepatic circulation. After the liver processes and conjugates estrogen for excretion, it is released into the gut via bile. In a gut with adequate beneficial bacteria and sufficient fibre, this conjugated estrogen binds to fibre and is excreted. In a gut with poor microbiome diversity, low fibre, and high beta-glucuronidase activity, the conjugated estrogen is de-conjugated — broken apart — and reabsorbed into the bloodstream before excretion.

This reabsorption increases circulating estrogen levels without any change in production. The gut microbiome community involved in this process is sometimes called the “estrobolome” — the collection of gut bacteria that metabolise estrogen. Women with higher beta-glucuronidase activity (associated with gut dysbiosis) have measurably higher estrogen levels.

The Constipation Connection

Slow gut transit — constipation — increases the time available for estrogen reabsorption before excretion. Women who are constipated have higher circulating estrogen levels on average than those with regular bowel function. Supporting the Estrobolome

Fermented foods: Yoghurt, kefir, sauerkraut, kimchi — these support microbiome diversity. A diverse microbiome is associated with lower beta-glucuronidase activity and better estrogen excretion.

Prebiotics: Food for beneficial bacteria — garlic, onions, leeks, asparagus, bananas, oats. Support the bacteria that keep beta-glucuronidase activity in check.

Reducing gut-disrupting inputs: Alcohol disrupts the gut microbiome. Antibiotics (when necessary) should be followed by deliberate probiotic support. Ultra-processed foods reduce microbiome diversity. These are all relevant alongside the positive additions.

A high-fibre, plant-diverse diet with fermented foods included regularly is the most practical gut health strategy for fibroid management.
