Waking with your nightclothes stuck to you and the sheet cold underneath. Getting up to change at two in the morning. And then lying there, wide awake, wondering whether this is normal.
Night sweats are one of the more distressing symptoms because they wreck sleep and because they carry a background fear — most people have heard they can signal something serious. That fear is worth addressing directly rather than talking around, so this article covers the hormonal causes, the non-hormonal ones, and exactly which features mean you should stop reading and make an appointment.
What is actually happening
Body temperature is regulated within a narrow band by the hypothalamus. Above the upper limit you sweat and dilate blood vessels to shed heat; below the lower limit you shiver and constrict.
The distance between those two limits is the thermoneutral zone, and in women with vasomotor symptoms it narrows dramatically. A small rise in core temperature that previously passed unnoticed now crosses the threshold and triggers a full heat-dumping response.
Two things make this worse at night. Core temperature naturally rises slightly in the second half of the night, and you are under a duvet. So the same physiology that produces a mild flush at three in the afternoon produces drenching at three in the morning.
Estrogen influences the width of that zone, which is why declining and fluctuating estrogen is the commonest cause in women over 40. But it is not the only one.
The hormonal causes
Perimenopause. The most likely explanation in your forties. Note that fluctuation matters as much as level — night sweats often arrive before hot flushes during the day, and before periods become obviously irregular.
Low progesterone. Less discussed. Progesterone supports sleep maintenance via allopregnanolone, so when it falls you wake more easily — and you notice sweating you would previously have slept through. Some women describe the waking coming first and the heat second, which fits this mechanism rather than a pure vasomotor one. The wider picture is in what low progesterone actually does.
Cycle-linked sweats in younger women. Temperature rises after ovulation under progesterone’s influence and drops before menstruation. Some women get sweats in the luteal phase or just before bleeding, entirely normally, decades before perimenopause.
Thyroid. An overactive thyroid raises metabolic rate and heat production — sweating, heat intolerance, palpitations, weight loss, anxiety. Underactive thyroid can also disturb temperature regulation. Both are simple tests, and the case for asking is in when a “normal” TSH is not the whole story.
The non-hormonal causes worth knowing
Alcohol. Probably the most common non-hormonal cause and the most commonly dismissed, because the drink was at seven and the sweating is at three. Alcohol causes vasodilation, then a rebound as it clears — sympathetic activation in the small hours. It also independently narrows temperature control. Two weeks without is the cleanest test you can run.
Nocturnal hypoglycaemia. If blood glucose drops overnight, the body releases adrenaline to correct it — and adrenaline causes sweating and waking. Signature: waking sweaty with a fast heartbeat, sometimes hungry, often after a carbohydrate-heavy evening or alcohol. Particularly relevant if insulin sensitivity has declined, as covered in the signs that appear before a blood test.
Obstructive sleep apnoea. Substantially underdiagnosed in women. Repeated breathing interruptions trigger sympathetic surges that cause sweating. Suspect it if you wake with a dry mouth or morning headache and feel unrefreshed regardless of hours slept. Risk rises after menopause, partly because progesterone supports airway muscle tone.
Medication. A long list, and easy to overlook. SSRIs and SNRIs are common culprits. Also some diabetes medications, hormonal treatments including tamoxifen and aromatase inhibitors, and paradoxically some drugs taken for fever. If yours started within a month of a new prescription, that is where to look.
Anxiety. Produces genuine sympathetic activation with real sweating. Not imaginary, and not the same as the vasomotor mechanism.
Your bedroom. Stated last because it sounds trivial and is genuinely responsible for a proportion of cases — synthetic bedding, an over-warm room, a partner who runs hot. Most bedrooms sit well above the 16–18°C usually recommended.
When it is not benign
This is the part people are looking for, so here it is plainly.
The pattern that warrants prompt assessment is night sweats together with one or more of:
Unexplained weight loss.
Persistent or recurrent fever.
Swollen lymph nodes in the neck, armpit or groin.
Persistent cough, particularly with blood.
Bone pain or unusual bruising.
Drenching sweats that require changing bedding and are entirely new, in someone not otherwise in the menopausal transition.
That constellation — sweats, weight loss, fever — is the classic set associated with lymphoma, and also appears with tuberculosis and some other infections. It is uncommon. It is also the reason not to spend a year assuming hormones if those features are present.
Isolated night sweats in a 46-year-old with shortening cycles and no other symptoms are a different situation entirely, and almost always hormonal.
Working out which is yours
Two weeks of notes will usually identify it. Record: whether you drank alcohol, what you ate in the evening, what day of your cycle you are on, and whether you woke hot or woke first and noticed heat second.
Follows alcohol → alcohol.
Follows carbohydrate-heavy evenings, with a racing heart → glucose.
Clusters in the second half of your cycle → hormonal.
Every night regardless, with morning headache → consider apnoea.
Started with a new medication → medication.
With weight loss or fever → appointment now.
What helps
Room at 16–18°C, cooler than most people keep it.
Natural fibres, layered. Layers you can shed beat one heavy duvet, and cotton or linen beat synthetics for moisture handling.
Separate duvets if you share a bed. Unromantic, effective.
Alcohol out for two weeks, properly out rather than reduced.
Protein-containing snack before bed if the glucose pattern fits.

On supplements for night sweats specifically: this is a category with a great deal of marketing and modest evidence. Black cohosh has been studied most and results are mixed. Nothing over the counter matches hormone therapy for vasomotor symptoms, and it would be dishonest to suggest otherwise.
