An ultrasound report noting a thickened endometrium alongside fibroids raises questions that many doctors do not adequately address. This article explains the connection, what the measurements mean, and when thickening warrants further investigation.
Normal Endometrial Thickness Across the Cycle
The endometrium changes thickness dramatically across the menstrual cycle. After menstruation it is at its thinnest — approximately 2–4mm. It thickens progressively through the follicular phase under estrogen stimulation, reaching 8–14mm around ovulation. In the luteal phase it remains thick, then sheds with menstruation. Any single measurement needs to be interpreted in the context of cycle day.
How Fibroids Cause Endometrial Thickening
The estrogen-dominant environment associated with fibroids directly stimulates endometrial proliferation. More estrogen, more endometrial thickening. Submucosal fibroids that project into the cavity also increase the effective surface area and alter the hormonal micro-environment of the endometrium. Women with significant fibroids often have thicker endometria at equivalent cycle points than women without fibroids — and this is one reason their periods are heavier (more lining to shed).
When To Investigate Further
A thickened endometrium measured in the correct cycle context (mid-to-late luteal phase) is often simply a reflection of the estrogenic environment. However: endometrial thickening over 14mm in the proliferative phase, or over 16mm in the late luteal phase, warrants further evaluation — particularly with sonohistogram or hysteroscopy to rule out endometrial polyps or hyperplasia.
Post-menopausal endometrial thickening above 4–5mm (depending on guidelines) always warrants further investigation, regardless of fibroid history. Post-menopausal bleeding with any endometrial thickening warrants urgent assessment. This is separate from fibroid-related thickening in premenopausal women — different threshold, different implications.
