Nattokinase and serrapeptase are two systemic enzymes that have gained attention in natural fibroid communities. The proposed mechanism — enzymatic breakdown of fibroid tissue — is appealing. But what does the evidence actually support?
What These Enzymes Are
Nattokinase: A protease enzyme derived from natto (fermented soybeans). It has well-documented fibrinolytic activity — it breaks down fibrin, the protein component of blood clots. This property is why it has been studied for cardiovascular applications including blood clot risk.
Serrapeptase: A proteolytic enzyme originally derived from bacteria in the gut of silkworms. Has anti-inflammatory and fibrinolytic properties. Used in some countries as a pharmaceutical anti-inflammatory, particularly for post-surgical swelling and sinusitis.
The Proposed Mechanism for Fibroids
Fibroids contain significant fibrin content. The theory is that systemic proteolytic enzymes, absorbed into the bloodstream, could break down this fibrin component, potentially reducing fibroid tissue. The biological logic is plausible — but the clinical evidence for fibroid-specific effects in humans is currently very limited.
What the Evidence Actually Shows
There are case reports and small observational studies suggesting improvements in women with fibroids using systemic enzyme therapy. There are no well-designed randomised controlled trials on nattokinase or serrapeptase specifically for uterine fibroids. Safety Considerations
Both nattokinase and serrapeptase have blood-thinning properties. Women with heavy fibroid bleeding should approach nattokinase with particular caution — its fibrinolytic activity could potentially worsen bleeding. Both interact with anticoagulant medications. Neither should be taken without discussing with your doctor if you are on any blood-thinning medication or have a clotting disorder. Avoid around surgery.
The Practical Position
Nattokinase and serrapeptase are reasonable additions to a broader supplement protocol for women who want to explore all available options and have discussed safety with their doctor — but should not be prioritised over the better-evidenced interventions.
